Undergraduate Students
This application for undergraduate admission will become part of your permanent record at Dakota Wesleyan University. To complete your application, please submit the following:
- Completed application and $25 application fee
- High school transcript, including class rank and cumulative grade point average
- Results of ACT or SAT
- Official transcript/s of all previous college work, if applicable
The following items are optional. No information you provide will be used in a discriminatory manner.
List chronologically all colleges/universities at which you have taken courses for credit in order of attendance.

Please list the adults who are legally responsible for you and/or have rights to your information.
If a minor, this is usually one or both biological parents when living.
Please give names and ages of your brothers or sisters.
If they have attended college, give the names of the institution and degree earned.
Describe any special circumstances and/or concerns which you believe
should be considered in connection with your application for admission.
Have you ever been found responsible for a disciplinary violation at any secondary or post-secondary school you have attended, whether related to academic misconduct or behavioral misconduct, that resulted in your probation, suspension, removal, dismissal, or expulsion from the institution?
Have you ever been convicted of a misdemeanor, felony, or other crime?
If you answered yes to either or both questions, please explain and give approximate
date of each incident and explain the circumstances.
I certify that all information in my application is my own work, factually true, and honestly presented. I understand the omission or misrepresentation of any information including enrollment in other colleges or universities is sufficient grounds for canceling my admission or registration.
The university reserves the right to deny admission or continued enrollment to any student who imposes an unreasonable risk of harm to the health, safety, welfare or property of the university, members of the university community or him/herself.
