Objective
To recognize an alumnus/alumna who has demonstrated outstanding service to Dakota Wesleyan University.

Criteria

• Graduate of DWU
• A person who provides outstanding service to the University.

Information to be submitted for Outstanding Service to Alma Mater Award

• Nomination form
• Describe how long, and in what capacity, you have known the nominee.
• Briefly describe the nominee’s major contributions to DWU – including significance and impact. Examples may include leadership in alumni events, promoting DWU, and/or contributions to DWU.
• Provide specific examples of the personal qualities that make this nominee an outstanding service leader.
• Recommendation letters that support the nominee are very helpful during the selection process; thus we would ask that the nominator request two others to submit letters of recommendation in support of the nomination. List the names and telephone numbers of those contacted to submit letters of recommendation.
  Note: Nominations can be submitted prior to the receipt of recommendation letters.
