In the event of injury or illness while competing or practicing
in athletics at DWU, the student-athlete must complete the following procedure:
- Report the injury to the athletic training department staff at DWU.
Non-referred treatment is NOT covered under DWU’s athletic insurance
policy. Failing to report an incident to the DWU Athletic Training staff
will result in the denial of all claims by the university’s insurance
carrier.
- Dakota Wesleyan Athletic Training staff, or the head coach of the
sport in the event of an emergency and absence of an athletic trainer,
will refer the athlete to the appropriate medical provider. Athletes
wishing to seek treatment by medical providers must communicate this
desire to the athletic training staff BEFORE such action is taken to
ensure insurance coverage is maintained. Those seeking second opinions
must be prepared to absorb all costs associated with the new medical
provider and can not assume Dakota Wesleyan’s insurance carrier
will help in payment of these bills.
Once the athlete has been cleared for competition by the attending physician,
all medical procedures related to the same injury will be the responsibility
of the athlete unless written approval is given by DWU’s sports
medicine personnel.
- All medical bills incurred by athletes from injuries sustained while
participating in intercollegiate sports at DWU must FIRST be filed with
the athlete’s primary insurance carrier. DWU’s policy acts
only as a secondary or excessive coverage policy.
- After the primary insurance carrier has made a determination of payment,
the athlete (or his/her parents/guardians) must submit a copy of the
primary insurance company’s Explanation of Benefits to the head
athletic trainer at DWU. This EOB will explain the amount that the primary
insurer has paid on the claim, applied to a deductible, etc. Please
forward all EOBs with copies of unpaid medical bills to:
Garry Grant, Head Athletic Trainer
Dakota Wesleyan University
1200 W. University Ave
Mitchell, SD 57301
- The head athletic trainer, and/or medical provider will then file
this claim with DWU’s insurance carrier, Student
Assurance Services, for consideration of payment on any unpaid balances.
- Currently, DWU’s policy carries a $1000 deductible that applies
to all injuries. This deductible must be met before payments from the
university’s carrier will begin. EOBs from all claims filled with
the university’s policy should be returned to the primary policy
holder showing the payment applications.
- Injuries that are sustained while not involved in intercollegiate
athletic practice or competition will NOT be covered under the university’s
athletic policy.
- All claims on athletic injuries must be filed within one (1) year
of injury in order to be covered by DWU’s athletic insurance carrier.
- Any questions or concerns regarding athletic insurance coverage or
policies at DWU should be directed to:
Garry Grant, Head Athletic Trainer
