Parent(s) or Guardian(s) Agreement:

I consent to the enrollment of my child(ren) in Dakota Wesleyan Daycare and agree that the Daycare shall not be responsible in case of illness or injury of the child(ren) while in attendance of the Daycare or in transit to and from the facility.

I give my consent for my child(ren) to take part in field trips or excursions under proper supervision.

I give my consent to having pictures taken of my child(ren) while at the Daycare and the pictures may be used as the Daycare so chooses.

I agree to carry out the rules and regulations of the Daycare as set forth in the Parent Handbook and other accompanying material.
