• coverage begins 1^st day of month after 1^st day of employment
• employees working 3/4-time or more are eligible
• $50 annual individual deductible
• $100 annual family deductible
• Schedule of benefit defines coverage
• member-dentist network

Employees should present their Delta Dental wallet cards to their dentists at each visit.
