DWU Medical Admission Requirements for International Students
Student Health Form. All incoming, returning or transferring students must complete the Student Health Form online. Log on to your TigerGo account, www.dwu.edu/tigergo, and follow the instructions under Student Health Form, letter “G”. Complete ALL sections of the form, and BE SURE TO LIST ALL MEDICATIONS THAT YOU TAKE, AND THE REASON YOU ARE ON THE MEDICATION. EXPLAIN ALL MEDICAL CONDITIONS IN DETAIL.
Immunization Documentation. Print the DWU Immunization form here. Your medical provider must complete the immunization form, and sign it with name, title, and address in the signature box at the end of the form. Unsigned immunization forms will not be accepted. Return the completed DWU Immunization form, and attach a copy of your official immunization record. You may scan and email the forms to Donna.Gerlach@dwu.edu, or mail the immunization requirements to:

Major Medical Insurance. Provide a copy of the medical benefits provided by your insurance company, the cover sheet that shows the start and end dates of your major medical insurance policy, and a copy of the front and back of your insurance card, if your company provides one. If your company issues an insurance card, go to www.dwu.edu/tigergo and follow the prompts to upload the card under Medical Insurance, letter “F”. If no card is available, DWU will manually enter the data when submitted by student. All insurance policies must be in effect for the entire semester, or academic year. If purchasing insurance by semester, updated paperwork will need to be submitted for the spring semester upon return. Determine the start date of your policy based on when your sport is required to report for practice.
If you are an athlete: You need a current physical for your collegiate sport. The athletic department requires a current physical be done on or after June 1 of the current year, and on file with the athletic department by July 1 of the current year for fall sport participation. For spring semester enrollment, physicals must be done on or after November 1 of the current year, and on file with the athletic department by January 1 of the starting year. PHYSICALS MUST BE DONE BY A MEDICAL DOCTOR, DOCTOR OF OSTEOPATHY, PHYSICIAN ASSISTANT, OR CERTIFIED NURSE PRACTIONER. Physicals can be done after arrival in the USA, upon student request for a cash fee. Print a physical form here. Scan, and email completed form to Marlene.Warnke@dwu.edu, and bring the original copy with you to turn in to athletics upon your arrival, or mail completed physical form to:
